Date Code AF (July of 1964).

Tall bottle envelope with a thin base, black staggered plates and a bottom square D getter.
The wording on the base is partially worn but still legible. The tube number is worn off of the glass.

Tested on a calibrated Hickok TV-10 military tube tester with no shorts and no leaks with the following results:

Gm Test = 2875-3000 micromhos
Minimum pass/good = 1650-1650

Also performance tested in-circuit through a Class A amplifier with no microphonics detected.
