Date Code 426 (June of 1954).
The 312 EIA code indicates that this tube was made by Sylvania for Emerson.

The wording on the base is faded but still legible. 
Top ridge of base has some small flea bite chipping but of course this doesn't affect performance.

Black angled T plates with clear mica spacers.

Tested on a calibrated Hickok TV-10 military tube tester with no shorts and no leaks with the following results:

Gm Test = 2625-2750 micromhos
Minimum pass/good = 1650-1650

Also performance tested in-circuit through a Class A amplifier with no microphonics detected.
