The tube has a radio shop's label on it dated July 7, 1950. 
The box shows wear, denting and is missing one small flap - see photo 4. See test results below.

Tested on a calibrated Hickok TV-10 military tube tester with no shorts and no leaks with the following results:

Gm Test = 1000 micromhos
Minimum pass/good = 700
Both diode sections test strong.
